The order of operations
Almost everything that goes wrong here comes from doing these in the wrong order:
- Evert the nipple. A nipple everter or silicone corrector, for a minute or two, immediately before the session. Gentle suction, released slowly.
- Measure it once it is out. A nipple measured while retracted gives you the wrong number. Measure after everting, at the widest point, in millimetres.
- Fit the flange to that measurement plus 0 to 3mm, per standard sizing guidance.
- Use the stimulation phase, properly. Fast and light until let-down, then switch to expression. This phase does more work here than raw vacuum.
Do steps one and two once, carefully, and most of the rest of the problem goes away.
Flat, retracting and inverted are not the same thing
- Flat. Sits level with the areola at rest, comes out with stimulation or cold. Usually the easiest case, and often needs nothing more than a minute of everting.
- Retracting. Comes out, then pulls back in when suction stops or when it is touched. An everter immediately before the session is exactly the tool for this.
- Truly inverted. Stays drawn in, and does not evert with stimulation. Worth an IBCLC appointment, both for latch help and because some cases benefit from tools and techniques we are not qualified to prescribe.
Which one you have changes the answer, and most people who assume they are in the third group are actually in one of the first two.
Why cycle control beats raw suction
The instinct is to buy the strongest pump available. That is usually wrong here. What draws a nipple out is a fast, light, stimulating rhythm — the pattern a pump's massage or let-down mode is imitating — not a hard sustained pull, which can flatten a nipple against the tunnel and make things worse. A pump that lets you adjust cycle speed and vacuum level independently gives you a chance of finding the combination that works for you. That is the single reason the Spectra S1 Plus leads this page rather than a stronger pump.
The friction problem, and silicone
An everted nipple that has just been drawn out is more susceptible to rubbing. Two things help:
- Silicone cushion inserts rather than bare hard plastic. Softer contact surface, and they double as a way to reduce the effective diameter.
- A slightly larger flange than the bare minimum, within the 0 to 3mm guidance, so the nipple is not touching the tunnel wall as it moves.
If you are also trying to nurse
Pumping and latching are different problems with a shared cause, and the everter helps both. Many people use a short pump session or an everter immediately before offering the breast, simply to give the baby something to attach to. That is a technique to run past an IBCLC for your situation rather than something to read off a product page — which is also our answer in how to choose a breast pump.
Worth saying plainly
We are not lactation consultants and nothing here is medical advice. Pain, bleeding or cracked nipples, a nipple that will not evert at all, or a baby who cannot latch or is not gaining weight are all reasons to see an IBCLC or your doctor rather than to buy a different pump. Everters use suction: apply gently, stop if it hurts, and release slowly. Clean everters and every part that touches milk per CDC breast pump hygiene guidance.

Common questions
Can you pump with inverted nipples?
Usually yes. Evert the nipple first with a nipple everter or silicone corrector, measure it once it is out, fit the flange to that measurement, and use the pump's stimulation phase before switching to expression.
Which breast pump is best for flat or inverted nipples?
The Spectra S1 Plus, because you can set cycle speed and vacuum separately in its massage mode, which is what draws a nipple out. A strong pump with only two presets gives you less to work with.
Does a nipple everter actually work?
For flat and retracting nipples it commonly does, for the few minutes you need. It is not a permanent change, and a nipple that will not evert at all is a reason to see an IBCLC rather than to press harder.
Should I measure my nipple before or after everting?
After. A retracted nipple measures smaller than it pumps, so sizing off that number gives you a flange that will rub. Evert, then measure the widest point, then add 0 to 3mm.
Will stronger suction pull the nipple out?
Not reliably, and it can flatten the nipple against the tunnel instead. A fast, light stimulation rhythm does the job that raw vacuum does not. Change the flange and the rhythm before you change the pump.
